Many of us, when considering long-term care, automatically assume “nursing home” is the option. Long-term care consists of many different options. Long-term care is the type of assistance someone needs on a regular basis due to a chronic illness, a physical and/or a mental disability. It generally consists of help with activities of every day living such as bathing, dressing and may include the services of a nurse.
In many instances, this type of care can be provided in the home by family members and friends or care providers. Sometimes, the individual may need more care and supervision than can be provided at home. For these individuals, there are a range of residential options available from adult homes to skilled nursing facilities.
Long-term care is a reality that many of us may face someday - either personally, or for one of our family members. Finding the right service can be difficult and confusing. In Cayuga County, there are a wide range of agencies, organizations or programs that provide information and/or assistance with long-term care services and community-based supports. Most people, when faced with decisions about long term care services, do not know where to go to find out about opportunities, services and resources available to help meet their particular needs.
The Cayuga County Long Term Care Access Office, which is part of the Department of Health and Human Services, assists people and their families with chronic care needs in order to maintain their independence and help them make informed choices about appropriate, cost-effective services. The Long Term Care Office is unique in the fact that it combines staff from both health and human services as well as staff from the Office for Aging. It serves as a single point of entry office where people can initially call to get information and assistance. It is located in the lower level of the Boyle Senior Center, 149 Genesee St., Auburn. The office's main telephone number is 253-1100.
Recently, you might have heard about or have seen the new logo advertising “NY CONNECTS: Choices for Long Term Care.” Although the local Cayuga County Long Term Care Office has been in existence for more than 10 years, it has received funding from the New York State Office for the Aging and Department of Health to market the office under the NY CONNECTS name and to further develop its services. The state has a vision of a restructured and accessible long term care system that supports the consumer's choice and independence and provides affordable services that meet their individual needs. The Long Term Care Office is basically the same as it has been, but now will be associated with the state's single point of entry initiative “NY CONNECTS.”
In one office, staff coordinate a number of county-run programs dealing with long-term care, including administration of most Medicaid homecare programs and the Office for Aging homecare program (EISEP). The office also offers Cayuga County residents assistance with planning for and obtaining cost-effective long-term care services for themselves or someone they know, regardless of age or income. Services include information and referral, intake and screening, and complete in-home assessment or evaluation of your needs.
Public Health nurses will conduct a free in-home assessment and develop an individualized care plan for any county resident who needs long-term care services. They also provide free screening (PRIs) for nursing home placement for County residents who need that level of care and aren't hospitalized. The office also provides a nurse liaison at Auburn Memorial Hospital to work closely with the social work office to facilitate discharges.
Individuals who contact our office are also offered information and referral services. Information and support is provided, as necessary, for individuals and their caregivers/families about all medical, non-medical (e.g. housing) and other community services; institutional services; eligibility criteria, etc. to link them with the wide range of services available to those in need of long-term care. Supportive services are also available to help caregivers cope with and understand their responsibilities. It also serves as a resource for long term care professionals and others who provide long term care services to older adults and to people with physical disabilities of all ages.
